FLORIDA NURSING LICENSE

Follow these simple steps to complete your Level 2 fingerprint background check requirement in Jacksonville FL:
1) Go to the Florida Department of Health website to obtain an ORI number and your Electronic Fingerprint Form:

3) The fingerprint and photo are captured by Fingerprint Chick Live Scan Jax and then sent to the Florida Department of Law Enforcement (FDLE). FDLE then sends the fingerprints to the FBI for their nationwide processing. The results are then disseminated back to the Florida Department of Health Clearinghouse. Once the results are received from FDLE, the average processing time for clearance is approximately 3-5 business days.
